Driver error blamed for truck accident

ESSEX — An investigation into a truck rollover off Western Avenue in Essex that sent a man to a Boston hospital has determined the rollover was due to operator error, according to Essex police. No charges were filed, and the case is closed.

The accident occurred last month when John Larry Ayers, 59, of Brentwood, N.H., who is a driver with Bentley Warren in Ipswich, was making a delivery of stone to Fisher Corp., a landscaping company at 7 Scot's Way. The truck rolled over while dumping the stone. Ayers was transported by a medical helicopter to Massachusetts General Hospital in Boston with non-life threatening injuries after firefighters extracted him from the truck's cab using a pneumatic extraction tool, commonly called the Jaws of Life. After an evaluation at the hospital, he was released.

The investigation was conducted by Essex police, the Massachusetts State Police truck team, the Massachusetts State Police accident reconstruction team, and the Occupational Health and Safety Administration.
